Microsoft OAuth doesn't pick up the avatar URL

After signing in via Microsoft, my profile shows the email-prefix initials instead of the Azure avatar. GitHub and Google work fine — Microsoft's picture claim is named differently.

Maps Microsoft/Azure picture claim into profile avatars (shared helper + callback backfill + DB trigger). Migration 0034_oauth_avatar_mapping.sql must be applied on deploy.
