Org logo preview: fallback when image URL fails

Severity: Low Area: Admin / branding
Org logo preview in settings hides broken images via visibility: hidden with no error text and no fallback to the letter tile.
After a bad URL or failed upload the preview area can look blank.
Fix: On onError, revert to letter tile and show inline validation message.
