Learning record: mode, concept and template score

Tasks

• Migration: job_outcomes.design_mode, concept, template_score, review_reasons (staff jobs only for text), redo_used boolean.
• recordOutcome fills them; Langfuse trace tags mode:<x>, concept:<y> and a score template_score.
• /v1/outcomes (staff) returns them; a simple staff view or query to compare modes and concepts (count, avg design score, avg template score, votes, deploy rate, cost).

Acceptance criteria

• Tests for toOutcome with the new fields; customer jobs keep numbers and labels only.
