Heal Enhance manage so live sites never land on Hosting not linked

Summary

• Opening website or domain manage now heals a missing Enhance website link from a same-domain sibling row or a live panel lookup, then loads the toolkit.
• Registrar domains that already live on Enhance (niletech.fi style) get hosting tools instead of an empty Hosting not linked page.
• Domain ids can be Enhance UUIDs or the hostname, including www vs apex.

This started from niletech domain manage showing Hosting not linked even though the site is on Enhance. The same heal covers future sites that exist on the panel but are not fully linked in OMA.

Test plan

• Open niletech.fi domain manage and confirm DNS / hosting tools load, not Hosting not linked.
• Open the linked website toolkit (domains, files, email) and confirm it loads.
• Open another Enhance site whose enhance_provisioned_service row is missing website id and confirm manage heals.
• pnpm test -- lib/enhance/provisioned-ref.test.ts lib/enhance/ensure-provisioned-ref.test.ts lib/registrar-domain-nav.test.ts in apps/web.
